It’s actually laughable how underdeveloped the rules around review are. I understand that some of that is an attempt to protect the refs ability to make decisions on the field, but this kind of thing is exactly the situation where there should be a review of it done without there even being a challenge flag thrown.
For a league generating $13 billion/year in revenue and paying out ~$400 million/year per team, there is a real lack of investment in actually running the games smoothly.

IMO it isn’t all about whether you have an explicit intent to hurt someone, rather it’s about using the wrong technique in a situation where you know that someone could get hurt. The defender gets paid a LOT of money to be one of the best at this sport, so there is a reasonable expectation that they will make every attempt to preserve their coworkers’ safety.
